2010 IMMUNIZATION CLINIC SCHEDULE
¨ All clinics are held by appointment only.
¨ Immunizations available to children (age 18 and under) are free.
¨ Vaccine for Children program eligibility applies.
¨ Children (under age 18) must be accompanied to the clinic by a parent or legal guardian.
¨ Immunizations available to adults are either free or have a fee, depending upon the vaccine.
¨ If you receive any type of Medicaid, please present your Forward card to clinic staff.
