GUIDELINES FOR ADMINISTERING MEDICATION TO STUDENTS
1. Students requiring medication at school shall be identified by parents/guardians to the building principal, who shall assume authority for involving designated school personnel in administration of the medication.
2. After identification of the student, the school nurse or properly appointed representative for the school shall make a parental contact to identify the type, dosage, and the purpose of medication.
3. The following criteria must be met in order for school personnel to administer medication:
a) Written statement from the parents/guardians (PARENT/GUARDIAN MEDICATION ADMINISTRATION CONSENT FORM), who shall request and authorize the designated personnel to give medication in the dosage prescribed by the physician, or as instructed by the parents/guardians for the over-the-counter medication. If more knowledge is needed by the school authorities to exercise prudent judgment for the safety and protection of the student on medication, permission should be obtained from the parents/guardians to contact the physician directly.
b) The prescribing physician must authorize all prescription medication to be administered during the school day. Natural products (herbal remedies, food supplements, vitamins, etc) of any kind will not be given during the school day unless the student’s physician authorizes its use and it meets FDA approval guidelines.
c) An emergency health plan must be completed for students requiring inhalers, glucagon, or epinephrine. This must be approved by the student’s health care practitioner.
d). Designated school personnel administering medications are required to complete the DPI medication training course. Only school personnel who have taken the course will administer medication to students.
4. The physician or pharmacist shall be requested by the parents/guardians to supply labeled bottles of medication for the school authorities for prescription medication needed during school hours. The label on the bottle shall contain the name and telephone number of the pharmacy, the student’s identification, name of the physician, name of drug, and the dosage to be given.
5. Medications, with the exception of medication that are self administered, shall be kept in a locked cubicle or other safe place at school. Taking medication shall be supervised by the school nurse or designated school personnel at a time conforming to the indicated schedule.
6. It is important that an accurate and confidential system of record keeping be established for each student receiving medication.
a) All medication forms will be stored in the office in which the medication is administered. The District Nurse will review the medications and forms monthly to ensure compliance with these procedures.
b) The classroom teacher and school personnel may be asked to record unusual behavior on medication.
c) An individual record for each student receiving medication shall be kept and will include the dosage, side effects, changes, continuance or disruption.
7. School personnel should under no circumstances provide ibuprofen, acetaminophen, sunscreen, or any other over-the-counter medication to students without meeting all the criteria above, with the exception of 3-b.
7. Special Provisions for Asthmatic Students – An asthmatic student may possess and use a metered dose inhaler or dry powder inhaler while in school, at a school sponsored activity or under the supervision of a school authority if all the following conditions are met:
a) The student used the inhaler before exercise to prevent the onset of asthmatic symptoms or uses the inhaler to alleviate asthmatic symptoms:
b) The student has written approval of the student’s physician and, if the student is a minor, the written approval of student’s parent or guardian; and
c) The student provides the building principal with the written approval (s)
8. Medication forms must be received annually for students on yearly medication.
9. Parents/Guardians must provide all prescription and over-the-counter medications to the school.
