Handy Health Information for Parents and Students in the W-F School District
Medications: If your child will be taking medication at school prescribed by a doctor, please remember these details: 1) Send the medication in the original container with the proper label (correct dose, time, and drug) 2) Have your child’s doctor sign the enclosed medication form authorizing school personnel to give the medication. 3) Fill out the rest of the form and sign the bottom.
If you are sending over the counter (non-prescription) medication to school, please fill out the enclosed form and bring it in the original container. A doctor’s signature is not necessary.
No medication will be given unless the medicine comes in the original container and appropriate paperwork is completed. All medication is kept in a locked compartment and will be given by trained staff. The only exceptions to this are emergency inhalers and glucagon.
A word on cough drops: Please do not send cough drops to school. Cough drops offer relatively no medicinal value and are a choking hazard, especially for young students. Please consider sending a water bottle and, if necessary, cough medicine. If your child’s cough is frequent, irritating, or productive, please keep him/her home for the day.
Health conditions: You will notice that each child attending school in the fall has a health update form for you to complete prior to registration. If any of your children have health conditions that may require health interventions (i.e. medications or procedures), please make sure that we communicate prior to school starting so that I can develop a plan of care for your child for the school day.
Immunizations:
These are the immunization requirements from the State of Wisconsin:
Pre K (2 years through 4 year-old kindergarten) 4 DTP/DTaP/DT 3 Polio 1 MMR 3 Hep B 1 Varicella (or history of chicken pox)
Grades K through 12 4 DTP/DTaP/DT/Td (One booster must be after the 4th birthday) 4 Polio 2 MMR 3 Hep B and 2 Varicella (or history of chicken pox)
(a student needs two doses if he/she did not receive the immunization prior to age 13)
***2 Varicella vaccines are needed for all students entering Kindergarten, 1st, ,6th7th,8th, 9th and 12th grades. A Tdap vaccine is required for students entering 6th- 12th grades.

Otherwise, make arrangements with your health care professional to ensure that these are done before school. Please send a record to the elementary office as soon as your child receives the shots. Meningococcal Disease Vaccine: Bacterial meningitis is an infection of the lining of the brain and spinal cord. A new vaccine has now been approved for people 11 and over and should seriously be considered. Please talk with your primary health care provider about this. As much as we would like to see your children in school everyday, there are times when it is best to consider taking the day off. Because we have lots of people in an enclosed space, it is difficult to control the spread of contagious illness in the school setting. We have implemented waterless hand washing stations in all kindergarten, first, second grade, and special ed rooms as well as in all cafeterias to deter germs from spreading. However, that is certainly not the cure all. The next page is a handy tool for you to reference if you are questioning whether your child is well enough to be in school.
